Reporting and Investigating the Injury
Note: All referenced forms are available in the Forms Section of the policy.
In Cases of Severe or Questionable Losses
- If the loss is severe or has very questionable circumstances, the Supervisor/Manager contacts the Univeristy Risk Manager, for investigation assistance. (612-625-0062)
- If the Supervisor/Manager has any questions about the compensability or legitimacy of the claim he/she should contact University of MN Workers' Compensation Department, who will consult with Sedgwick. (612-624-6019)
Managing the Injury
Note: If the Supervisor/Manager receives any medical bills from the employee they should forward them on to Sedgwick.
Returning the Employee to Work
It is the Supervisor/Manager's responsibility to follow up with the injured employee for the Work Status Report Form within 24 hours of the initial doctor's appointment.
No Restrictions
- If there are no restrictions and no lost time, the Supervisor/Manager should fax the Workability Form to Sedgwick (952-826-3889).
With Restrictions
- If there are work restrictions that are easily understood and met, the Supervisor/Manager should put the employee back to work. He/she should then fax the original Workability Form to Sedgwick.
- If the Workability form provides the injured worker with work restrictions which, are not easily met, the Supervisor/Manager should contact Sedgwick.
- It is the Supervisor/Manager's responsibility to follow up with the injured employee if they are working with restrictions or are totally off work.
- If the employee is working with restrictions the Supervisor/Manager should contact that employee at a minimum of once a week to determine how the employee is doing and when the next office visit is. The Supervisor/Manager must obtain the Workability Form within 24 hours of any subsequent doctor appointments and should forward this form to Sedgwick.
No Release to Return to Work
- If the Workability Form does not return the employee to any form of work, the Supervisor/Manager will immediately contact Sedgwick for assistance.
- It is the Supervisor/Manager's responsibility to follow up with the injured employee if they are working with restrictions or are totally off work.
- If the employee is missing work, the Supervisor/Manager should contact that employee once a week to monitor the situation.
