Report and Investigate the Injury.

• The employee should report the injury to you and complete the Employee Incident Report.

• Provide the employee with a copy of the Workers' Compensation Information for the University of Minnesota Pamphlet; the Employer's Authorization for Care and a Work Status Report Form.

• Complete the First Report of Injury Form (FROI) within 24 hours of the reported injury.

• Complete the Supervisor Incident Investigation Report within 24 hours of the reported loss.

• Fax the FROI to Sedgwick Claims Management Services (952-826-3785) within 24 hours. The original should be forwarded to the University of MN Workers' Compensation Department.

• Send the Employee Incident Report and the Supervisor Incident Investigation Form to the University of MN Workers' Compensation Department.

• The University of MN Workers' Compensation Department will make copies of the Employee Incident Report and Supervisor Incident Investigation Form and forward the copies to Employee Health and Safety. The originals will be filed in the University of MN Workers' Compensation Department.

In Cases of Severe or Questionable Losses

• If the loss is severe or has very questionable circumstances, you should contact the Univeristy Risk Manager, for investigation assistance at (612) 625-0062.

• If you have any questions about the compensability or legitimacy of the claim you should contact University of MN Workers' Compensation Department, who will consult with Sedgwick at (952) 826-3800 as well as Greg Casura (Environmental Health & Safety) at (612) 626-6002.
